If you are a provider who has at least one Jefferson Health Plans Dual Special Needs Plan (D-SNP) member assigned to your practice, at least one person on your staff who is involved in their care must complete our annual D-SNP Model of Care training module. This training is required by the Centers for Medicare & Medicaid Services (CMS).

Our 15-minute DSNP Model of Care Training is available at JeffersonHealthPlans.com/provider-online-courses.

To maintain compliance with CMS requirements, you must complete and attest to the completion of the training by December 10.
