We value our partnership with you and the care you provide to our members—your patients. Each year, we survey our provider network to assess compliance with Access and Appointment Standards and Telephone Availability Standards. These standards are established by the Department of Human Services (DHS) for Health Partners Plans Medicaid and CHIP, and by the Centers for Medicare & Medicaid Services (CMS) for Jefferson Health Plans Medicare Advantage.
Completion of this survey is mandatory for all participating primary care providers and select specialist provider groups.
Surveys were faxed to mandated provider offices on April 10, 2026, and include a survey ID and a link to complete the survey. One survey must be completed for each survey ID, which corresponds to each provider office location.
Please complete the survey by May 8, 2026.
