Diabetes Prevention Program

Have you been told you’re at risk of Type 2 diabetes? If you have, help is available, and Jefferson Health Plans EverWell gives you access to the Diabetes Prevention Program.

Health Education Advisory Committee (HEAC)

Jefferson Health Plans EverWell has a Health Education Advisory Committee (HEAC) that is comprised of our Medicaid members, employees and community members. This committee meets to share ideas about our programs and services and let us know how we can do things better.

HEAC meets four times per year. Members of the committee may be eligible to receive $50 for every meeting they attend.

Third-Party Liability (TPL)

As a Medicaid member, you may also have Medicare or other health insurance. This other insurance is known as Third-Party Liability (TPL) and is considered your primary insurance.

Where can I find my benefits? 

You can find a list of your benefits on our Jefferson Health Plans EverWell Medicaid plan page or in your member handbook. If you have any questions, our Member Relations team is here to help. Call us at 1-800-553-0784 (TTY 1-877-454-8477). 

I have questions about my plan. Where can I find answers?

If you have any questions, your member portal is a great place to start. Here, you can order a new ID card, change your provider, access personalized benefits information and more.

Where can I find more wellness resources? 

Our Community Wellness Centers located around the region host cooking, fitness and life skills classes and community education workshops. All Community Wellness Center events are free and open to the public. Learn more by going to our Community Engagement page.

Where can I learn more about covered and non-covered services? 

Your member handbook will have the latest information on covered and non-covered services, plus benefit information. The member handbook can be found here.
