Health Partners Plans Medicaid and CHIP Rebrand Under Jefferson Health Plans

Transition brings all Medicaid, CHIP, Medicare Advantage, and Individual & Family plans under one cohesive Jefferson Health Plans identity.

PHILADELPHIA, Pa., June 1, 2026 – Jefferson Health Plans today announced the next phase of its system-wide rebrand, bringing all Health Partners Plans (HPP) products under the Jefferson Health Plans name.

Health Partners Plans Medicaid is now Jefferson Health Plans EverWell, and Health Partners Plans CHIP is now Jefferson Health Plans CHIP, unifying all plans under a single, cohesive identity.

Following Jefferson’s acquisition of HPP in 2021 and the initial rebrand launch on July 1, 2023, the transition continues through 2026. The organization’s Medicare Advantage and Individual & Family plans have already been operating under the Jefferson Health Plans name since 2023.

“Bringing all of our plans together under one name strengthens our identity and reinforces our commitment to the communities we serve,” said Joseph Cacchione, MD, CEO, Jefferson.  “While our name is changing, our mission remains the same: to improve lives by removing barriers to care and supporting the health of every member.”

Members will continue to receive the same coverage, benefits, doctors, and services they rely on today, and both members and community partners will continue to work with the same trusted teams.
