Health Partners Plans Name Change to Jefferson Health Plans CHIP
What changed?
Health Partners Plans CHIP is now called Jefferson Health Plans CHIP.
When did this occur?
The name change took place on June 1, 2026.
Will I still have the same benefits?
Yes. Your child’s coverage, benefits, and services stay the same. We will continue to offer programs that support their wellbeing and our teams will continue to help you navigate their health care.
Will we need to pick a new doctor?
No. Your child can keep seeing the same doctors and providers, as well as others that are part of our large and growing network.
Will we receive a new ID card?
Yes. You should receive a new Jefferson Health Plans CHIP ID card in the mail within the month of June. Until then, keep using your child’s current ID card for their care. Both ID cards are valid through 2026.
Why is the name changing?
Our Medicaid and CHIP plans have joined our Medicare Advantage and Individual and Family plans under the Jefferson Health Plans name.
Is the “Health Partners Plans” name going away?
Yes. We will soon stop using the Health Partners Plans name. But there may be a period when you see both the Health Partners Plans and the Jefferson Health Plans names. They refer to the same plan.
